New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.
The Practice covers:
- Redcar
- Marske & New Marske
- Wilton, Lazenby & Yearby.
We are unable to accept patients outside of this area.
Patients wishing to join will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from their previous doctor.
New patients may be offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication they may be taking.
Once registered, patients may book appointments with the GP of their choice subject to availabilty.
New Patients Prescribing Policy
The doctors in this Practice only prescribe short courses of the following drugs in exceptional circumstances due to their addictive nature:
- Temazepam
- Nitrazepam
- Lormetazepam
- Diazepam
- Lorazolam
- Zopiclone
- Zolpidem
If you wish to join the Practice and are currently taking one of more of these drugs you will be asked to consent and participate in a planned reduction programme - you will receive full support from the Practice.
Change of Name/Address
Please let us know of any changes to your name, address and any additions to your household so that our records can be kept up to date.
